Abstract

Abstract

Background: Pediatric brain arteriovenous malformations (AVMs) frequently present with hemorrhage and carry a mortality rate of 5%–10%. This report evaluates the management of two pediatric cases involving different therapeutic pathways based on clinical complexity.

Case Presentations: Case 1: An 11-year-old boy presented with isolated headaches, vomiting, lethargy, and slurred speech. Preoperative MRI/CT diagnosed an AVM of the anterior cerebral artery. The patient underwent primary endovascular embolization using SQUID-1 via an Apollo microcatheter. Post-procedural angiography confirmed complete occlusion of the AVM. At discharge, the patient showed improved sensation in the left extremities.

Conclusion: Successful management of pediatric AVMs requires multimodal strategies, including endovascular embolization and microsurgery, tailored to the Spetzler-Martin grade and rupture status. Objective outcomes in these cases demonstrated that combining liquid embolic agents with surgical excision can achieve complete obliteration and stable neurological recovery.
